PYLN Secondary Transition Toolkit 2012
The Pennsylvania Youth Leadership Network (PYLN) has created secondary transition toolkit to assist youth in their transition into the adult world. The best thing about the toolkit is that it was written and designed by youth with disabilities who are members of the Pennsylvania Youth Leadership Network (PYLN) for youth. It can also be used with families and professionals as a youth-friendly Toolkit for transition.

Successful Health Transitions (Effective Practices)
As youth and young adults transition from school to adult life, planning based on health concerns is an integral aspect and is closely intertwined with success in post-secondary education, independent living, and community living goals. Success in these arenas requires that youth be as healthy and independent as possible in managing their health and medical related conditions. This webinar will address the importance of planning for health care needs in the secondary transition process; special concerns and community resources for particular complex health conditions; and provide examples of “best practices” in health planning in the transition process.
